I consent to receiving mobile phone text messaging from Washington County WIC which may include the transmittal of information that is personal, confidential and/or is protected by HIPAA.  This includes but is not limited to:

·  Information related to the scheduling of meetings or other appointments

·  Information about general WIC services

·  Breastfeeding support and services

Due to circumstances beyond the control of Washington County WIC,  I understand that there is a risk to receiving this information by text.  This includes, but is not limited to:

· Persons other than you having access to your mobile phone.

· You do not password protect your mobile phone.

· Your mobile phone is lost or stolen and you do NOT report the incident to Washington County WIC.  Washington County is not liable for texts sent to your mobile phone if it is not in your possession.

· You change your mobile phone number and do NOT report this to Washington County WIC.  Washington County is not liable if the texts are sent to the mobile number we have on file for you.
